Playboy has named the first openly transgender female as a Playmate. A French model named Ines Rau.

Unless he had some unusual genetics, I am not sure what type of operations they did to change his hips and such.  Typically that is the dead giveaway.  I can still see some male structure in the face, not so much elsewhere (from the site, anyway).
