Looks like that form might only be good if you can buy it at Blain’s Farm & Fleet.  The offer is good at lots of places including:
Cycle Gear
and
Orchard Supply Hardware

Those are the only two near me anyway.

I think that form can be used at any of the 'participating' stores. If you look at the fine print it tells you what stores it can not be used at. At the Mobil site it lists the stores that are participating.

Thanks Conrad!  It's time for new oil anyway and who doesn't like to save their green!
