Yeah- emphysema. He had been in deteriorating health due to that for some time and was using a wheelchair for a while now- nothing wrong with his legs, just an unfortunate side- effect of disappearing lungs. He quit smoking a long time ago but COPD can appear long after one stops smoking.

Funny thing but I cannot picture Mr. Spock with a cigarette....
